Patient-Centered Primary Care and Medical Homes: Defining Models for the Future of Care Delivery

Health care reform ultimately aims to build a more sustainable health care system by driving innovation that leads to better quality of care, better patient experience, and lower costs. Patient-Centered Medical Homes (PCMH) are one particular care delivery model the industry has seized on as a key tool to help drive change. PCMH model pilots administered by the Centers for Medicare and Medicaid Services and private insurers have already proven to positively impact quality and cost of care. Much has been learned in the past decade about how best to establish these innovative care delivery platforms, and much is still being learned as they evolve and the initial successes wear off.

As organizations across the nation explore different ways to incorporate PCMH models into their strategy, the concept is being tested in a variety of settings and structures with the same shared goal of coordinating better and less costly care. The faculty for this program will address which elements are essential to any PCMH model and how this model is being used to meet health critical care reform objectives.

Ken Yale, Clinical Solutions for ActiveHealth Management
Ken Yale, Vice President of Clinical Solutions for ActiveHealth Management, is responsible for business development and supporting product development for a diverse set of innovative clinical decision support tools. Prior to ActiveHealth, he served as chief executive of an innovation business unit in AmeriChoice, the Medicaid division of UnitedHealth Group.

Dr. Yale's previous work in medical management and innovation includes Corporate Vice President at Matria Healthcare, head of Government Programs for CorSolutions, and principle consultant with Health Solutions Network. He also led business and product development at EduNeering, a web-based training company, and was President and CEO of Advanced Health Solutions, a healthcare consulting firm.

Before building health technology businesses, Dr. Yale held leadership positions in the federal government. From 1989 to 1992 he was Chief of Staff of the White House Office of Science and Technology and served as Special Assistant to the President and Executive Director of the White House Domestic Policy Council (DPC). Prior to his White House duties, Dr. Yale served as Legislative Counsel in the U.S. Senate for the Senate Leadership, a Commissioned Officer in the U.S. Public Health Service, and instructor at Georgetown University.

Dr. Yale earned a Bachelor of Arts from Creighton University, a dental doctorate from the University of Maryland, and a J.D. from Georgetown University.

Brian Parker, Aetna
Aetna’s Patient Centered Primary Care (PCPC) business is focused on radically improving the quality and cost-effectiveness of the care received by millions of Americans nationwide by equipping primary care physicians (PCPs) with advanced healthcare information technology (HIT) and sophisticated clinical and operational services. As a comprehensive “payer-agnostic” solution, PCPC will enable PCPs to successfully and economically migrate from the current episodic and fragmented care model to the preventive and whole person care model as exemplified by the patient centered medical home (PCMH), resulting in dramatically improved quality and cost effectiveness for all patients.

The PCPC business is led by Brian Parker, an executive with extensive experience building and leading service businesses in the physician office setting. Before joining Aetna in May of 2011, Brian spent over eight years at US Oncology, a $4B cancer care services company. While at US Oncology, Brian built a $600M business providing independent community-based oncology practices with comprehensive bio-pharmaceutical services via a nationwide team of oncology pharmacists and admixture technicians. Brian was also responsible for commercializing iKnowMed, an oncology specific Electronic Health Record system, Oncology Reimbursement Solutions, a revenue cycle management business, and Innovent Oncology, a physician directed evidence based medicine disease management business focused on improving the quality and cost-effectiveness of cancer care during his tenure with US Oncology.

Brian is passionate about the Patient Centered Primary Care business and the potential to make a significant impact in the lives of patients nationwide while improving the US healthcare system via the PCMH care model.

Hardship Policy
